Membership Registration:

 Submitting this form means that you are making personal commitment to make an annual donation of $400 ($100 at each quarterly meeting), to local Huntington charities. Even if the charity chosen is not your first choice, you are committing to donate at each meeting.  If you are not able to attend the quarterly meeting, you should make arrangements to submit your check to your team leader either before or immediately after the meeting.  You may send a blank check for $100 with your team leader.
